VACCINE CLINIC PARTICIPATION REQUIREMENTS

Please read the information below prior to clicking on the “Schedule a Vaccine Clinic Appointment” tab at the end of the page so that you are aware of our participation requirements and arrive at our clinic fully prepared. Thank you.

APPOINTMENTS ARE REQUIRED AT ALL OF OUR CLINICS (Except for our once monthly La Puente Vaccination/Wellness clinic which is a walk-up clinic, no appointment is needed).

OUR CLINIC RULES & PROTOCOLS:

* Pet owners from any city or county may attend any of our clinics.

* Pet owners must be at least 18 years of age to participate. Minors must be accompanied by an adult.

* By California law, pets must be at least 3 months old to receive a Rabies vaccine, and any pet receiving its initial Rabies vaccination, regardless of age, can only receive a 1-year Rabies vaccine.

* We are able to administer a 3-year Rabies vaccine only if you show valid proof (a Rabies Vaccine Certificate signed by a Veterinarian licensed in the U.S.) that your pet is at least 15 months old and received its first Rabies vaccine at least one year (365 days) prior to our clinic date and that your pet is not more than 3 years overdue for a Rabies vaccine booster.

* All pets over the age of 3 months will receive a Rabies vaccination at our clinic unless the pet is current on its Rabies vaccination and proof of that vaccination is provided at the time of pet’s appointment. Valid and acceptable proof is a Rabies Certificate signed by a veterinarian licensed to practice in the United States.

* Bring other prior vaccine records if possible. This will enable us determine which vaccines are needed or recommended for your pet at this time and their next due dates. We do our best to ensure that pets are properly vaccinated and also not “over” vaccinated.

* Fearful or unfriendly dogs must wear a muzzle prior to entering our clinic line or coming in close proximity with another animal. A properly fitting basket muzzle is recommended, as other types may cause your dog to overheat. If you do not have a muzzle, a restrictive muzzle will be provided for your use only during the time our staff is handling your dog.

* Dogs must wear a properly fitting collar or harness so that your dog is secure and cannot slip out of its collar/harness, especially when pulling backwards. A sturdy leash must be attached to the collar/harness. No chains or expandable leashes are permitted. In the event your dog needs better control, for the safety of your pet and for others nearby, we may require you to place our slip lead on your dog while at our clinic.

* All cats must be in secured carriers designed to transport cats or small dogs. No cardboard carriers are allowed.

* If your pet receives a microchip, do not bathe it for 2 weeks after the microchip is implanted. The microchip enrollment fee has been pre-paid by HEART; however, it is your responsibility to register your pet on line or by phone and keep your contact information current with the respective microchip company. Your pet cannot be reunited with you if it becomes lost and if you cannot be found!

* Nail trims and anal gland expressions are provided only if we have the extra time and only for pets which are cooperative and easy to manage.

* Prescription flea & tick control medication is sold in quantities of 1 month to 12 month supplies (depending on the stock on hand). We do not provide prescriptions.

* Please be courteous to everyone and reschedule your appointment if you or household members have shown any signs of illness within 10 days prior to your appointment.

* You will receive a yellow copy of a Certificate of Vaccination before you leave our clinic, along with other informational materials such as post-clinic care instructions and a list of emergency pet hospitals. You are responsible to keep your copy of the Certificate. A $25 processing fee will be charged for any duplicate Certificate provided.

WE CANNOT VACCINATE PETS WHICH:

* Are too aggressive to be handled, especially cats. If your pet is likely to be difficult to handle, please do not schedule an appointment at our clinic, as we are limited on time that we are able to spend with each pet and must refuse service for any pet which cannot be handled with minimal restraint.

* Have ever had an adverse reaction to a vaccine (other than mild soreness or mild lethargy).

* May have shown any sign of illness within the past 2 weeks;

* May be pregnant;

* Are nursing puppies/kittens that are less than 4 weeks old;

* May have certain underlying health conditions or are on certain medications which, when administering a vaccination, may pose a health risk to the pet.

TO SCHEDULE A VACCINE APPOINTMENT:

Deposits Required

All appointments require a $22 deposit per pet which is credited towards the services rendered at the clinic. Deposits are paid at the time you submit your appointment request through our website and are NON-REFUNDABLE unless you cancel no later than 24 hours prior to the date of your appointment by emailing us notice of your request to cancel your appointment at: [email protected]. Write “CANCEL APPOINTMENT” in the subject line of that email. Non-compliance with the aforementioned terms, regardless of mistake or error on your part or for any other reason, will result in forfeiture of your depositBy submitting your appointment request and your deposit, you agree to the terms set forth in this paragraph.

Time Slots

You will schedule an appointment “time” through our website app; however, that time actually reflects the start time of a 30-minute “window” / “time slot” in which you must arrive. (For example, if you schedule an appointment for 7:00 pm, you must arrive between 7:00 and 7:30 pm.) Multiple pets are scheduled during each time slot. To help reduce long lines and waiting times, we will not take pets earlier than their scheduled time slots, so please do NOT attempt to check in sooner than your scheduled time.

Multiple Pet Appointments

If you need to schedule an appointment for a cat and a dog, please schedule both in a time slot designated for cats.

If you are bringing more than one pet, you must schedule each pet appointment individually and separately.

In the event you have multiple pets and there are not enough available openings in one time slot, OR if your pet is in a vaccine “series” and must receive its 2nd or 3rd vaccination within a 2-6 week period after its last vaccination and there are no available appointments, please call us at (714) 993-9193 and we will do our best to accommodate you.

If the appointment scheduler will not allow you to make an appointment, then no openings are available at this time; however, we do receive cancellations up to 24 hours in advance of the clinic, so please check back frequently.

In the very rare instance that a clinic must be cancelled due to harsh weather conditions or other dangerous or emergency situation, a notice will be posted on our website Calendar as soon as it is known that the clinic must be cancelled. We will also attempt to contact everyone to provide the opportunity to reschedule their appointments to another day. In the event we are not able to contact you, please contact us.
